New Physical examination guide lines and information

IMPORTANT: CHANGES TO MSHSAA PHYSICAL EXAM REQUIREMENTS

The Missouri State High School Activities Association now allows a physical examination by a medical professional to be valid for a duration of two years (730 days) from date of issue listed on the physical.  However, some physicians continue to practice a 1-year policy.  Make sure that you are aware of what your physician marks for your student’s participation approval (1 year or 2 year).

AT STALEY, WE WOULD PREFER IF ALL STUDENT-ATHLETES WERE ON THE FOLLOWING CYCLE:  ONLY incoming freshmen and incoming juniors should get physicals this spring and summer.  That would mean student-athletes will get a physical to cover freshman and sophomore years and then a physical to cover junior and senior years.

If you have any questions, please contact the Staley Activities Office at 816-321-4254.

IMPORTANT: CHANGES TO COLLECTION OF REQUIRED PHYSICAL FORMS

Staley High School is no longer using PRIVIT to electronically sign and store all documents necessary for MSHSAA student-athlete eligibility purposes.

Rather, Staley’s Athletic Office will be collecting the 4 pages necessary to participate in MSHSAA sanctioned activities and athletics. See details below.

MSHSAA Physical Forms (5 pages)
Take all of these forms with you to the doctor
Parent completes pages 1 & 2
Doctor completes pages 3 & 5
Turn-in page 5 to the school

MSHSAA Pre-Participation Documentation (3 pages)
Parent and student complete and sign all 3 pages

Turn-in pages 1-3 to the school

All-together, the school collects 4 pages from the above documents.  You can bring these 4 pages to your coach/sponsor prior to the start of your season or send them to the following email staleyphysicals@gmail.com.  Staley’s Athletic Office will make sure to file them and provide them to the staff as needed.
